In 1997 it was estimated that 90,000 patients died in US hospitals due to medical errors. Unfortunately, very little progress has been made since that time and in fact new problems have emerged.

Many have compared the complexity of the airline industry to that of healthcare and have wondered why the airline industry has been so successful in preventing unexpected adverse events and the healthcare industry has not. However, the comparison between these industries is superficial. As an illustration, see linked figure, which provides an example of the magnitude of a patients physiologic relational complexity and the various potentially deadly failure modes for a single typical patient. Yet nurses often care for 12 or more such patients at the same time. It's analogous to having one pilot fly a dozen poorly maintained aircraft through different treacherous storms at the same time. So, it's not surprising that the most common cause of preventable death in the hospital is uncontrolled complexity. Most hospitals are fully aware of this problem but simply do not have viable solutions to solve for them.
